VNA Health Group is seeking a Registered Nurse (RN) Home Health Clinic for a nursing job in Brick, New Jersey.

\nOverview:\n

Responsible for providing comprehensive nursing services to women and their families eligible for the Nurse Family Partnership program in Ocean County.

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Attends weekly team meetings, case conferences and clinical supervision meetings
• Assesses and evaluates needs of women and their families as they relate to health and life course development
• Carries a caseload of up to 25 clients and completes home visitation in accordance with the NFP model elements and NFP Visit-to-Visit guidelines
• Provides counseling and instruction to assist women and their families to attain targeted goals
• Develops and maintains community relationships to support client referrals
• Precepts and mentors new hires and makes visits with student nurses as needed
• Adheres to agency infection control and safety policies
• Adheres to the organization’s policy in regards to absenteeism and appearance
\n\nQualifications:\n
• Bachelor’s degree in Nursing
• 2-3 years work related experience
• Strong computer skills
• Excellent verbal and written communication skills
• Excellent interpersonal and organizational skills
• Bilingual- Spanish speaking a plus
